Magee Rehabilitation, the rehabilitation arm of the federally designated Regional Spinal Cord Injury Center of the Delaware Valley, provides life time follow-up services for persons with SCI. Despite all efforts, these services tend to be primarily reactive in nature and often fail to prevent readmissions. The SCI literature and a baseline survey of past patients showed that as many as 55% of newly injured patients will be readmitted within a year of their injury. The goals of the specialized medical home include reducing readmissions and emergency department visits and improving quality of life of persons with SCI. The program accomplishes this through the delivery of integrated, coordinated, and proactive healthcare services. This presentation will highlight results from more than three years of services.
Describe the model of a proactive, specialized, multidisciplinary SCI Medical Home to ensure successful short and long term outcomes for patients and families with complex spinal cord injuries and chronic disability.
Discuss how the financial and psycho-social challenges faced by these patients compound the medical issues and the ways that this model proactively addresses those issues.
Identify lessons learned from this model project, from both the administrative and clinical perspectives.
