Upon admission to the inpatient rehabilitation hospital, the patient and family members are overwhelmed with new information and the road ahead.  In a quest to improve the patient experience and overall patient outcomes throughout the rehab journey, Rehabilitation Institute of Michigan (RIM) put it in reverse, and looked both ways at a tool that had been used for patient education and realized the tool was at a dead end and needed to take a U turn for a Road to Recovery. The tool was a Patient Education Notebook which contained valuable information; however, the information was not user friendly, was not visually appealing, did not focus on the patient, and the notebook was not being used as an interactive patient education resource book for the patient and/or family members. The first stop light on RIM's road to recovery was to talk with their Patient & Family Advisory Council about the Patient Education Notebook.  It was at that meeting that it became apparent that the tool was either put in a closet and never looked at, or the patient's family was reading it and didn't understand the materials.  The RIM team then started the transition from the Patient Education Notebook to the Road to Recovery Patient Resource Guide. This session will review the Road to Recovery tool, and the process in which RIM transitioned from an educational notebook to an interactive patient resource guide, the impact on the patient, the impact on staff communication/ interactions, the impact on the discharge process, and the impact on patient experience scores.

  • Express how preparing the patient upon admission to their rehab journey (with a patient centric tool) will enhance patient experience outcomes.
  • Recognize how an interactive educational tool can enhance the patient experience.
  • Explain how the use of a patient resource guide can improve team and patient communication to improve the discharge process and patient outcomes.
