Have you ever thought acute care hospitals refer appropriate IRH/U patients to a SNF or competitor because it is easier and quicker than waiting for an admission decision from your IRH/U?  If you answered yes, you might have a problem.  In today's value-based purchasing environment, assuring efficiency of key referral and admission processes impacting a patient's acute hospital journey to inpatient rehabilitation is critical for maximizing census and developing collaborative partnerships that facilitate smooth transitions between care facilities.  Acute care hospitals are increasingly forming preferred provider networks that focus on facilitating smooth transitions, preventing unnecessary readmissions and reducing the acute care length of stay.  This presentation will focus on identifying the common barriers to an efficient referral/admission process, strategies for removing the barriers and maximizing census.
