Title: Assessing Return to Acute Care for Program Improvement

This course is designed to be a resource to inpatient rehabilitation leaders, nurses, occupational therapists, physical therapists, case managers, social workers, physicians, and speech language pathologists related to the care of the inpatient rehabilitation patients in order to reduce their risk of acute care readmission. This course will review the quality reporting requirements for inpatient rehabilitation for of acute readmissions and the changes to be implemented October 1, 2018. The presentation will review research based methods to identify patients at risk of readmission, suggest ways of individualizing your facility’s analysis of patients at risk for readmission, and present forward thinking solutions for performance improvement methods. The scope of the presentation will provide key information for those new in rehab as well as innovative practices for advanced programs.
At the conclusion of this course, participants will be able to:
- Recognize the quality reporting requirements currently in place for acute care readmissions and prepare your facility for changes beginning October 1, 2018
- Identify key research that differentiates patients at risk for readmission within inpatient rehabilitation programs and review evidence-based methods used to reduce acute care readmissions
- Analyze your facility and strategically implement progressive and innovative performance improvement solutions for new and advanced programs
